Asian eyelid surgery, also called double eyelid surgery, builds a stable crease between the eyelid skin and the tissue beneath it. The surgeon anchors skin to the tarsal plate or the levator aponeurosis, the tendon that lifts the lid.

In many East Asian eyelids the orbital septum attaches lower, the preaponeurotic fat pad sits fuller, and the levator muscle sends fewer fibers toward the eyelid margin. Not a defect. - Choosing crease height. Measured from the lash line, most creases fall between 4mm and 8mm. A 4mm to 5mm crease reads soft, a 6mm to 7mm crease gives clear definition, and a high fold at 7mm to 8mm is the most visible and the one that most often looks operated when set wrong.
- Choosing crease shape. A tapered crease, or infold, converges toward the medial canthus. A parallel crease, or outfold, runs toward the lateral canthus and widens the eye. The ideal fold follows your own lid function.

The Procedure: Incisional, 30 to 60 Minutes

- Weeks 2 to 4. Healing continues, residual asymmetry settles, and light exercise resumes. Social activities are comfortable for most patients.
